It is also the responsibility of the patient to confirm that our doctor is included in your insurance plan.
Referral Guidelines
- Call Primary Care Provider (PCP) before your appointment.
- Tell them the date of your appointment.
- Let them know the name of the doctor at our office you are seeing.
If your insurance requires you have a referral you must have one before seeing the doctor or your insurance will deny
your claim.
Payment Guidelines
patients are responsible for their insurance deductibles. Co-pays must be paid on the day of service.
